Connecticut is ranked last in the country in hospice length of stay which translates into last minute hospice and limited end-of-life care planning.  However, there is good news…The state has begun a series of major end-of-life planning initiatives to enhance patient and provider knowledge and decision-making.  As a result, we hope to begin to improve timely utilization to appropriate end-of-life care. Such work also requires ongoing and concerted efforts among organizations all along the care continuum. So many providers frequently encounter situations in which patients and families struggle to make end-of-life choices mainly because they don’t know what their choices are.  This presentation will provide options, based on The Conversation Project, for providers across the care continuum to consider as they encounter patients with serious illness nearing the end of life.
