Our CT Department of Public Health continues to share ongoing trends concerning the lack of care coordination when home health and hospice providers service residents in Assisted Living Facilities. With this in mind, the CT Assisted Living Association and the CT Association for Healthcare at Home are once again offering this collaborative, interactive morning program to learn ways to optimize coordination of care provided by home health or hospice agencies within Assisted Living Facilities while maintaining regulatory compliance.   Back by popular demand, legal expert Heather Berchem (Murtha Cullina LLP) will provide the foundation of the regulatory requirements (both federal and state) for each provider type.  Following the regulatory presentation, we will engage the audience in valuable and practical panel discussions to share best practices and address challenging scenarios. 

Objectives

  • Understand the federal and state regulatory requirements for Assisted Living Facilities, Home Health Care and Hospice Agencies
  • Utilizing panel provider discussions, learn ways to optimize care coordination across provider types.   
  • Address complicated care scenarios using examples offered by the participants.
