Join us for a morning review of the Department of Social Services (DSS) home and community-based service delivery system and the status of DSS' Strategic Rebalancing Plan.  Presenters Kathy Bruni and Dawn Lambert (Co-Leaders of the DSS' Community Options Unit) will lead an interactive discussion and offer participants the opportunity to ask questions and provide feedback on the status of change initiatives within the state.

Objectives:

1) The different home and community-based waivers available in the state;

2) The differences in eligibility requirements associated with the waivers as well as any waiting list or cost cap limitations;

3) New services available under waivers;

4) The status of the state's strategic rebalancing plan;

5) Additional initiatives currently in progress including:

        Community First Choice, No Wrong Door, Universal Assessment
