The home health agencies’ star ratings were made available to the public on Home Health Compare in July, and only 2.6% of 9,000 agencies measured earned a top score of 5 stars. Now, more than ever, agencies need to place even more focus on coordination of care to improve process and outcome measures as CMS uses these to determine star ratings. Lower ratings can have a direct effect on referrals. Don’t lose referrals to other higher ranked agencies. This session will describe what two agencies did to stand out on key processes and outcome measures. Participants will receive an understanding of proven strategies to assist in improving quality outcomes and star rating.



  • Identify strategies to maintain or improve your star rating
  • Review best practices to improve publicly-reported process and outcome measures, including medication reconciliation
  • Review proven strategies for reducing hospital readmissions, including how to ensure patients visit their physicians within the first week after hospital discharge
  • Explain how to implement QA strategies that ensure patients do not return to the hospital
  • List resources & services to offer patients that will help with patient satisfaction, while also reducing the likelihood of falls or medication mishaps
