Reducing Falls and Managing Function: Creating Meaningful Outcomes

Last year, the OIG identified a trend in Home Health regarding underreported falls with major injury and hospitalizations.  Home Health Value-Based Payments also emphasize the impact of quality outcomes based on accurate reporting of behaviors, dyspnea and GG functional OASIS items.  Join us as industry expert and physical therapist Cindy Krafft connects the dots between these industry challenges and trends to help home health providers attain optimal outcomes based on intentional care planning and accurate OASIS data collection which ultimately impact agency HHVBP outcomes and STAR ratings. In addition, falls prevention best practice strategies and documentation will be shared to address and help mitigate the trends identified through the OIG.

Objectives

Attendees will understand:

  1. How to attain optimal outcomes through intentional care planning based and accurate data collection;
  2. The impact of quality outcomes based on reporting behaviors including current HHVBP (dyspnea) and 2025 HHVBP (GG functional items);
  3. The importance of accurate falls data collection, documentation, care planning and reporting along with best practices to mitigate falls with injury.
