Care Coordination and NEW Intake/Referral Safety Requirements for Home Health: Working Together Across the Care Continuum A Program for Assisted Living, Home Health and Hospice Care Coordination between licensed care settings is a regulatory requirement yet an ongoing challenge for Home Health, Hospice and Assisted Living Facilities (ALFs). Also, effective October 1st, all referrals to home health will now require additional intake data elements seeking to mitigate risk and ensure the safety of home health visit staff. The CT Association for Healthcare at Home is once again collaborating with the CT Assisted Living Association to offer a half day morning session to review the Care Coordination regulations, the new intake/referral requirements for home health and to engage the attendees in discussion of strategies to optimize compliance and communication in both areas. - Our morning will kick off with legal expert Heather Berchem (Murtha Cullina LLP) who will provide the foundation of the regulatory requirements (both federal and state) for care provision in an ALF by Home Health or Hospice.
- Tracy Wodatch (Pres/CEO of the Connecticut Association for Healthcare at Home) will then provide an overview of the new intake laws for home health as outlined in Public Act 24-19. Guidance resources for compliance will also be shared.
- To encourage optimal collaboration, a panel discussion comprised of home health, hospice and ALF staff will share best practices, offer sample resources and address challenging scenarios.
