We are a nonprofit full-service Medicare-certified, licensed home health care and hospice agency that is part of McLean Affiliates' life plan community, post-acute and long term care continuum located in Simsbury, Connecticut .
Our mission is to provide high quality, effective, and compassionate multidisciplinary services to home health and hospice patients in our local communities.
We serve adults over age 18 and specialize in care of elders in the Farmington Valley, Bloomfield and West Hartford.
All of our staff are well-trained employees, not contracted persons, and we enjoy a 98% retention rate among our seasoned staff. We provide skilled nursing (RNs), telemonitoring, home health aides with special hospice and dementia training, medical social work, physical therapy, occupational therapy, speech therapy / speech language pathologist, plus spiritual counseling, special volunteer services, bereavement programs, and Hospice Medical Director oversight for our hospice patients and families.
We provide palliative home care and hospice consultations to help you and your family understand comfort-focused care and your choices when coping with serious acute or chronic conditions.
Our highly experienced, board-certified hospice professionals help you and your family live fully and comfortably, supporting you along life's journey.
We are a Medicare Five Star Agency per the first CMS release in July 2015, among only 2.6% of agencies in the nation to earn this high score. We have also earned High Satisfaction Scores posted on Medicare's Home Health Compare and national Home Care Elite status.
