Western Connecticut Home Care (WCHC) is a non-profit, Medicare certified and State licensed medical home care agency. The Agency is the home care affiliate of Western Connecticut Health Network, which includes Danbury Hospital, New Milford Hospital and Norwalk Hospital.
Western Connecticut Home Care provides comprehensive nursing, therapy, medical social work and home health aide services to patients with a variety of health conditions using the latest in home care technology throughout the Greater Danbury, New Milford and Norwalk areas.
Specialty services are offered for chronic disease care and management, fall prevention assessment, geriatric care, infusion therapy, Lifeline personal emergency response system, maternal child health, nutrition therapy, orthopedic rehabilitation, palliative care, tele-monitoring and wound care.
