Western Connecticut Home Care (WCHC) is a non-profit, Medicare certified, State licensed, and Joint Commission accredited home health agency. The Agency is the Connecticut affiliate of Northwell Health servicing most cities/towns of Western Connecticut.
Western Connecticut Home Care provides comprehensive nursing, therapy, medical social work and home health aide services to patients with a variety of health conditions. Specialty services are offered for disease care and management, post-surgical care, fall prevention assessment, geriatric care, infusion therapy, maternal child health, nutrition therapy, orthopedic rehabilitation, palliative care, and wound care.
