Healthcare at Home welcomes you to apply for membership in one of our seven membership categories:
Licensed Home Health and
Hospice Provider Agency
Membership is available to an entity that is licensed by the State of Connecticut as a home health care agency
and certified by Medicare as a home health hospice agency.
Please note: Related agencies that have more than one
Connecticut state license, but who share the same governing authority for the purposes of state licensure, will
be considered as one member.

Affiliate (Non-Medical)
Home-Based Care Provider
Membership is available to an entity that provides home care services and does not otherwise qualify for
membership as a Licensed & Certified Provider.
Please note: An Access Agency would qualify as an Affiliate
Provider Member.

Associate Membership for:
Business
Membership is available to a business or organization who provides products and/or services directly
or indirectly related to home health, hospice or community-based care and is not a direct provider
of home care services.

Sole Proprietors
Membership is available to an individual who owns an unincorporated business by him or herself and
does not elect to treat a domestic limited liability company (LLC), as a corporation. The business
owner and/or consultant provides products
and/or services directly or indirectly related to home health, hospice or community-based care and
is not a direct provider of home care services.
Please note: The company can operate under the name of its owner or can do business
under another name.

Not-for-Profits
Membership is available to an Organization or Association whose services relate
directly or indirectly to home health, hospice, or community-based care, but is not a direct
provider of home care services.

Health Care
Provider
(Non Home-Based-Care)
Membership is available to a provider of health services with an interest in home health, hospice or
community-based care and is not a direct provider of home-based care services. Example: Hospital,
ACO, Physician Practice, etc.

Membership is available to an individual who is not (1) directly employed by a
member organization or (2) does not otherwise qualify for membership, including (but not limited to)
retirees, full-time students and Board Member of Association
Members.
