MODULE 3- Person-Centered Home Health Care Management

Health care reform is requiring better care resulting in better health at a lesser cost (the Triple Aim). Learn how expert person-centered care coordination, case management and interdisciplinary communication in home health are the keys to achieving the Triple Aim. Nurses and therapists will collaborate in this all-day session to hone their case management skills using real case examples as well as documentation, reimbursement and regulatory information learned from session one.
Goals & objectives to include:
- Reinforce important regulatory issues affecting home care
- Describe key elements of the intake/referral process
- Identify components of the admission, revisit, recertification, resumption of care, and discharge processes
- Describe the importance of concepts such as health literacy, motivational interviewing, chronic care management, palliative care and person-centered care
- Using case examples for both nursing and therapy, apply multidisciplinary case management and care coordination focused on person-centered care planning
